Care That Starts From the Condition, Not a Generic Checklist
Specialized home care is in-home support shaped around a specific health condition. The plan reflects how the condition affects movement, energy, eating, medications and safety, and it is reviewed as the condition changes.
Freiheit Care supports people living with Parkinson’s disease, recovery after stroke, multiple sclerosis and other neurological conditions, diabetes, heart and lung conditions, and the effects of cancer treatment. A registered nurse oversees the care, nursing care can be added where it’s needed, and we assign the same caregiver wherever possible, so the person who is in your home knows your routine.

- NOTRE ENGAGEMENT
What Makes Specialized Care Different From General Home Care
It starts with a conversation about the condition. We ask what your doctor or care team has recommended, what a good day and a hard day look like, and what worries the family most.
A registered nurse plans and reviews the care. The caregiver is briefed on the condition, your routines and your care team’s instructions before the first visit, and nursing care is added where clinical monitoring is needed.
The plan changes as the condition does. Needs rarely stay still. We review regularly and step care up or down, with no long-term contract.
- What a Night Actually Looks Like
We follow your care team’s lead. Specialized care supports your doctor’s and therapists’ plan at home; it doesn’t replace it.

What Specialized Care Can Include
Every plan is different. These are the building blocks we put together around your condition and your care team’s recommendations.
A Care Plan Built Around Your Condition
A registered nurse reviews your situation and your care team’s recommendations, and the plan is built and reviewed around them rather than from a generic checklist.
Mobility Support and Fall Prevention
Help moving safely around the home, getting up and down, and managing stairs, with attention to the stiffness, weakness or balance changes that come with the condition.
Medication Reminders and Routines
Reminders to take medications as prescribed and at the right times, and support keeping to the routines your care team has set.
Nutrition and Meal Preparation
Meals that suit your dietary needs and are easy to eat safely, prepared the way you like them, so eating well doesn’t depend on having the energy to cook.
Appointments and Therapy
A caregiver accompanies you to medical and therapy appointments and tells your family how it went, so instructions aren’t missed.
Nursing Care Where Needed
Registered nurse oversight is part of how we work, and nursing care can be added where clinical monitoring or wound care is part of your plan.
- Conditions We Support
Specialized Home Care for Common Complex Conditions
Here is what care at home often focuses on for the conditions families ask us about most. Your plan is
always shaped by your own situation and your care team.

In-Home Respite
A caregiver comes to your family member’s home for personal support, meals, companionship and, where needed, nursing. It can be arranged publicly through Ontario Health atHome if approved, or privately through an agency like ours. It suits people who are most comfortable in familiar surroundings.

Adult Day Programs
Supervised group activities at a community centre for a day or part of a week. Publicly funded programs need a referral through Ontario Health atHome, and most charge a fee for each day attended. They suit people who enjoy company and structure, and spaces can be limited, so it’s worth asking early.

Short Stay in a Long-Term Care Home
A temporary stay for people with higher nursing needs when the caregiver needs to be away for more than a day, is in hospital, or is unwell. Stays are limited to 60 days at a time and 90 days per calendar year, and the person pays accommodation costs. These are arranged through Ontario Health atHome.
Many families use more than one — for example, an adult day program on weekdays and in-home respite on evenings and weekends. We’ll tell you honestly which fits your situation, including when it isn’t us.

Specialized Home Care for Common Complex Conditions
Here is what care at home often focuses on for the conditions families ask us about most. Your plan is
always shaped by your own situation and your care team.
Parkinson’s Disease
Safe movement and fall prevention, help with dressing and bathing when stiffness or tremor gets in the way, medication reminders at the right times, and meals that are easy to eat safely.
Stroke Recovery
Help with mobility and personal care, reinforcing the routines your rehabilitation team has set, and accompaniment to therapy and follow-up appointments.
MS and Other Neurological Conditions
Flexible support that adapts to good days and hard ones, with attention to fatigue, balance and energy, and care reviewed as needs change.
Diabetes
Meal preparation that follows your dietary plan, reminders to keep to medication and testing routines, and attention to foot and skin care. Nursing care can be added where needed.
Heart and Lung Conditions
Pacing daily tasks around fatigue and breathlessness, help with meals and household tasks, and a caregiver who notices and reports changes to family and our nursing team.

When Families Call Us About Specialized Care
Living at Home With a Chronic Condition
Daily tasks take more energy than they used to. A caregiver who knows the condition keeps life at home safe and manageable.
A Recent Diagnosis or Change
A new diagnosis or a step down in ability can leave families unsure what help to arrange. We help you work out what’s needed now, and what may be later.
A Family Stretched Thin
Caring for a parent or spouse with a complex condition is a lot to carry alone. Care fills the hours your family can’t, and gives you a break.
A Family Managing From a Distance
Adult children in another city can have a caregiver in the home and clear updates, instead of piecing things together over the phone.
People Who Want to Stay Independent
The goal is to keep doing as much as possible for yourself, with the right support for the rest.

- Who Specialized Care Is For
Signs That Support at Home Needs to Step Up
With a chronic or complex condition, needs often change gradually. These are common signs that it may be time to add support or talk to your care team.
- Falls, near-falls or new difficulty with stairs and transfers
- Missed or confused medications
- Weight loss, skipped meals or difficulty eating
- Daily tasks like bathing and dressing becoming harder
- A family caregiver who is exhausted
- Appointments being missed or hard to get to
- Knowing When to Ask for More Help
For urgent symptoms, contact your care team, and call 911 in an emergency. Our caregivers let your family and our nursing team know about changes they notice. This is general information, not medical advice.
How Specialized Care Is Paid For in Ontario — Including the Options That Aren’t Us
Several routes are worth knowing before you commit to anything.
Publicly funded home care through Ontario Health atHome. If you’re eligible, nursing and personal support at home are covered through OHIP, at no cost to the family. A referral from your doctor, specialist or hospital discharge planner is the usual route, or you can call them at 1-833-515-1234. The trade-offs are real: hours are set by the program, so they may be fewer than you need, visit times are assigned rather than chosen, caregiver consistency isn’t guaranteed, and there can be a wait. If you haven’t been connected yet, ask your doctor or care team about a referral before you pay for anything. We’ll tell you the same on the phone.
Family-Managed Home Care Program. Some households also qualify for the Family-Managed Home Care Program, which lets eligible families direct their own funding to a provider of their choice — Freiheit Care is an accepted provider for households who qualify.
Insurance. Some extended health and workplace insurance plans cover part of the cost of home care. Check your policy, and we’ll provide an itemized receipt for any claim you submit.
Veterans Affairs Canada. Eligible veterans can receive funded care through VAC. We’re an approved provider and handle the coordination and paperwork directly.
Private specialized care. What Freiheit Care provides. You choose the schedule, you get a caregiver matched to your needs, and RN oversight comes with it. Many families use publicly funded services as a base and add private care on top, particularly for the hours and continuity that public schedules cover less well.
Visits start at four hours, because building a real relationship and providing unrushed, dignified care matters to us. We give you a transparent, itemized quote after the free consultation, with no hidden fees and no long-term contract.
- Straight Answers on Cost
If publicly funded care will meet your needs, we’ll say so. We’d rather be the agency you trust than the one you resent.
Complex Conditions Need Someone Who Knows Your Routine
A caregiver who returns learns how you move, how your energy changes through the day, how you like things done, and what a normal day looks like, which means they notice when something is different. A new face each visit starts from scratch every time, and with a complex condition that is the last thing you want to explain again.
We assign the same caregiver wherever possible, and you have a say in who that is and when they come.
Caregiver turnover is the industry’s biggest quality problem, and the reason is pay. In May 2026, Freiheit Care was certified as a Champion Level Living Wage Employer by the Ontario Living Wage Network — the highest tier in Ontario. We pay above the living wage, because that’s how you keep good caregivers long enough for consistency to matter.
- Why the Same Caregiver Matters
Fair pay → the same caregiver stays → they know your condition → changes get noticed early.

- QUESTIONS FREQUENTES
Specialized Care in Ottawa — Your Questions Answered
Specialized home care is in-home support shaped around a specific health condition, such as Parkinson's disease, stroke recovery, multiple sclerosis, diabetes, or heart and lung conditions. It goes beyond general help with daily tasks: the care plan reflects how the condition affects movement, energy, eating, medications and safety, and a registered nurse oversees it.
We commonly support people living with Parkinson's disease, recovery after a stroke, multiple sclerosis and other neurological conditions, diabetes, heart conditions such as heart failure, lung conditions such as COPD, and the effects of cancer treatment. Dementia care is covered on its own page. If your situation isn't on this list, call us and we'll tell you honestly whether we can help.
Personal care covers daily tasks like bathing, dressing and meals. Specialized care starts from the condition: what changes day to day, what to watch for, and what your care team has asked for. A registered nurse builds and reviews the plan, the caregiver is briefed on it before the first visit, and nursing care can be added where clinical monitoring is needed.
Our caregivers are vetted, insured, bonded, police-checked and trained in senior care. Before care begins, a registered nurse reviews your situation and briefs the caregiver on your condition, your routines and your care team's instructions. We'll be clear about what a caregiver can do and where nursing care is the better fit.
Yes. Registered nurse oversight is part of how we work, and nursing care can be added where it's needed, for example for clinical monitoring or wound care. A caregiver covers daily support, and the nurse reviews the plan and notices when it needs to change. Tell us what your care team has set up and we'll recommend the right mix.
Private specialized care is priced hourly, depending on the hours you need and whether nursing is involved. We give you a transparent, itemized quote after the free consultation, with no hidden fees and no long-term contract. Publicly funded home care through Ontario Health atHome is free to eligible families, and some households use the Family-Managed Home Care Program or an insurance plan toward private care.
Ontario Health atHome provides publicly funded nursing and personal support for eligible people, covered through OHIP. Eligibility and hours are decided by Ontario Health atHome, and hours may be fewer than a complex condition needs. Many families use public services as a base and add private care for the hours and continuity they don't cover.
Ask your family doctor, specialist or hospital discharge planner to refer you, or call Ontario Health atHome yourself at 1-833-515-1234. A care coordinator checks eligibility and plans services. It's worth doing this before paying for anything privately, and we'll tell you the same on the phone.
Yes. Care for Parkinson's typically focuses on safe movement and fall prevention, help with dressing and bathing when stiffness or tremor makes it harder, medication reminders to keep to your prescribed schedule, meals that are easy to eat safely, and companionship. Needs change over time, so we review the plan regularly with the family.
Yes. After a stroke, care at home often means help with mobility, personal care and meals, reinforcing the exercises and routines your rehabilitation team has set, and support with communication and confidence. A caregiver accompanies you to follow-up and therapy appointments. We follow your care team's instructions and review the plan as recovery progresses.
Care is reviewed regularly and adjusted as needs change, up or down, with no long-term contract. Our caregivers let your family and our nursing team know about changes they notice, so questions are raised early. For urgent symptoms, always contact your care team, and call 911 in an emergency.
We assign the same caregiver, or a small consistent group, wherever possible. With a complex condition, a caregiver who knows your routine, your limits and what a good day looks like is the one who notices when something is different.
Visits start at four hours, because building a real relationship and providing unrushed, dignified care matters to us. There is no long-term contract, so you can adjust or stop care as your situation changes.
Yes. The consultation is free, by phone, and carries no obligation. Tell us about the condition and what your care team has recommended, and we'll tell you honestly whether and how we can help, including if public services or another option might suit you better.
