Freiheit Care Inc. (“Freiheit Care,” “we,” “us,” or “our”) provides in-home care services across Ottawa and the surrounding region. Because of the nature of our work, we collect and handle personal information—including personal health information—about the clients we serve, their families, our website visitors, and our staff. This policy explains what we collect, why, how we protect it, and what rights you have.
This policy complies with Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA) and Ontario’s Personal Health Information Protection Act (PHIPA), which governs how health information custodians like home care providers handle personal health information.

What Information We Collect
From website visitors: Name, email address, phone number, and any details you submit through a contact or consultation request form. We also collect standard website usage data (pages visited, browser type, approximate location) through cookies and analytics tools—see “Cookies We Use” below.
From clients and their families: Contact and identification details; health information relevant to the care plan (medical history, diagnoses, medications, care needs, mobility, and cognitive status); emergency contact information; and billing or funding information, including details relevant to Veterans Affairs Canada, Ontario Health at Home, Family-Managed Home Care, or private insurance claims where applicable.
From caregivers and staff: Employment application details, certification and licensing information, background check and vulnerable sector check results, and payroll information.
Why We Collect It
We collect and use personal information to provide and coordinate care services; communicate with clients and families about scheduling and care plans; process billing and funding claims; comply with regulatory and professional obligations for our nursing and personal support staff; respond to consultation requests and general inquiries; and improve our services and website. We do not sell personal information to third parties, and we do not use health information for marketing purposes.
Who We Share Information With
We share information only as needed to deliver care and run our business, including with the caregivers and nurses directly assigned to a client's care; funding bodies such as Veterans Affairs Canada or Ontario Health at Home, where a client is receiving funded care; insurance providers, for claims a client has authorized; and service providers who support our operations (for example, payroll or scheduling software providers), under confidentiality obligations. We do not share personal health information with any other party without consent, except where required or permitted by law—for example, to report a risk of serious harm or in response to a valid legal order
How We Protect Your Information
We use physical, administrative, and technical safeguards appropriate to the sensitivity of the information involved, including restricted access to client records, staff confidentiality agreements, and secure storage of physical and electronic files. No system is completely immune to risk, and we continually review our practices to reduce it.
How Long We Keep Information
We retain personal health information for the period required under Ontario regulations governing health records and other personal information, and for as long as needed for the purpose it was collected, after which we securely destroy or anonymize it.
Your Rights
Under PIPEDA and PHIPA, you have the right to know what personal information we hold about you or your family member, request access to your records, request correction of inaccurate information, and withdraw consent for certain uses of your information, subject to legal and contractual restrictions (for example, information required for an active funding claim).
