Our dedicated and experienced Nurse Manager help you create your own #1 personalized home care package based on your health needs and unique goals.

After an initial free home assessment and in depth discussion of your needs and desires, we will tailor a bespoke package to ensure that you are indeed free to live.

FREE TO LIVE INDEPENDENTLY

Bob lost his wife last year and although he wants to stay in the family home he’s finding it a bit too big to manage on his own. He doesn’t want to admit it, but he is worried that he is becoming increasingly forgetful. He doesn’t want to lose his independence.

After his home assessment with Freiheit Care, Bob has started cognitive stimulation activities with his support worker and together they have designed subtle memory cards for around the home. He now has help with laundry and cleaning as well as meal preparation so the house isn’t overwhelming any more. Bob also gets regular wellness checks to help him be proactive about his health. He feels in control and independent.

LIVE FREE WITH SUPPORT

Sheila has fallen a few times at home and her husband Jim is worried that she is becoming increasingly frail. His own health is deteriorating and he isn’t able to support Sheila as much as he would like. They both love their neighbourhood and don’t want to move but it is becoming harder to cope.

The Nurse Manager has advised weekly physio and strength building exercises so that Sheila recovers fully and reduces the likelihood of future falls. The nurse has also reviewed their diets and medication and made some suggestions to improve both their health. As Sheila is concerned about slipping in the bath, a personal support worker is helping her with this and other personal needs on a weekly basis. The fact that they are now getting help with light house work and maintenance means that Sheila and Jim now feel confident living in their own home. They feel supported.

LIVE FREE WITH COMPREHENSIVE CARE

Charlie is recovering after a stroke and although Sarah, his wife, is fit and healthy, she can’t leave him alone and they both want him to get the best care to aid his recovery. The comprehensive support they get from Freiheit Care means he was discharged from the hospital knowing that they would get all the care they need. He is receiving physio, occupational therapy and speech and language pathology to aid his recovery.

The nurse is regularly monitoring his blood pressure and vital signs and working directly with other primary healthcare providers. This proactive management is designed to prevent future hospitalization and ensure Charlie is always on the right medication.

They also signed up for regular respite care so that Charlie’s wife can continue to attend her book club and go grocery shopping etc. Charlie attends social events organized by Freiheit Care which also allows Sarah to meet up with friends and maintain her own independence. This comprehensive care means that both Sarah and Charlie are free to live their lives and support each other.
